What is an education, health and care plan?
An education, health and care plan (EHCP) describes the child or young person and what aspirations they and their family have.

The EHCP sets out what the needs of the child or young person are and how to meet those needs. It records desired outcomes and steps to achieving those outcomes.
The EHCP is made up of 12 sections. The council for Disabled Children have created a guide, which shares what type of information should be included in each section.
From Year 9 onwards the plan should include information and provision to prepare for adulthood.
