Parental declaration information
Parents and carers are required to sign a parental declaration form when taking up the funded early education offer.
The declaration form gathers the information the local authority requires to be able to pay the fees to the childcare setting. It sets the terms and conditions that applies to the parent and carer for taking up the funded early education place.
The information provided on the form allows the childcare provider to make a claim for the following additional funding, if applicable:
- Early Years Pupil Premium (EYPP)
- Disability Access Fund (DAF)
